Dear Parents/Guardians of Arlee Schools Students,
On Wednesday, April 30, 2025, the Lake County Health Department will be offering Tetanus, Diphtheria, and Acellular Pertussis (Tdap) vaccinations to the 6th graders at Arlee Schools.
We are also offering Meningococcal (MCV4) and Human Papiloma Virus vaccines (HPV/Gardasil9) to all eligible students. The Advisory Council on Immunization Practices (ACIP) strongly recommends these vaccines for both boys and girls.

There you will find the consent form and Vaccine Information Statements (VIS) and will choose which immunizations you would like your student to receive. You will be asked to provide insurance information; however, no student will be turned away due to inability to pay. Please complete this form by Tuesday, April 29, 2025.
